Problems with the Exhaust Adapter (Modified)
Mod Note 46, TCTO 562, EEM 6345.1
We have presently deployed Modification Note 46 (a.k.a. TCTO 562 and EEM
6345.1) Retrofit of the Exhaust Adapter in the Kohler Engine Generator. The
purpose of this modification is to remove and replace the exhaust adapter in the
exhaust system of the Kohler engine generator with a new modified exhaust
adapter. We have been notified there are problems with the replacement unit at
some sites.
Problem:
Two sites have reported trouble installing the new modified exhaust adapter. Both
sites returned the problem units to the ROC for analysis. What we have found is
the rings exceed the dimension tolerances of the specification. The incorrect ring
dimensions are why the unit either won't fit or is a very tight fit in some instances.
We are unable to determine when the change occurred in the manufacturing
process. Therefore it is unknown how many sites are affected.
Solution:
We would like any site having trouble installing this unit to halt installation.
Reinstall the unmodified exhaust adapter to restore the generator to proper
working order. Notify the WSR-88D Hotline immediately so we can identify the affected
site. If you have installed the unit with force (brute strength or hammer) it will
have to be removed and be identified as a defective unit. Do not discard the
modified unit, hold it for further instructions.
If you have not installed the unit, please continue with plans to install. Twenty-two sites have reported no problems with the installation. We apologize for the
inconvenience and plan to correct all reported problems expeditiously.
